Client Rights & Information

At Sugar Land Wellness Center, we believe in transparency, respect, and empowering our clients with clear information about their rights and available resources. The information below is provided in accordance with Texas law and is available to support you throughout your care.

Requesting Your Health Care Records

Clients have the right to request access to their mental health records.


To request your health care records, please submit a written request to Sugar Land Wellness Center by:

Requests are processed in accordance with Texas law. Additional authorization or documentation may be required prior to the release of records.

Filing a Consumer Complaint

If you wish to file a consumer complaint regarding services received, you may do so through the Texas Office of the Attorney General.
